High-5 & Kindergarten Registration!
Welcome to Sullivan STEAM Magnet School! The teachers, support staff, and I are looking forward to working with you and your child during the 2022-2023 school year. We will have two full-day high-5 & three full-day kindergarten classrooms this coming fall and are looking forward to seeing new faces and welcoming returning ones.
Please come in sometime before June 24th during school registration hours of 9:00AM to 1:00PM to complete your registration.
To complete the registration process, we will need you to bring the following information to Sullivan:
- BIRTH VERIFICATION (birth certificate, passport, baptismal record, or immigration card)
- RECORD OF IMMUNIZATIONS (including immunizations required for entry to Kindergarten)
- Recent physical exam, if available.
- Emergency contact information (names, addresses, phone numbers)
- Information on childcare arrangements (address, phone number), for transportation purposes (if needed).
We will need all the items listed above before your child begins school. if your child attended preschool at Sullivan last year, we have this information on file, though you may need to bring an updated immunization record and copy of their 5-year physical exam.
