High Five and Kindergarten Registration
We will be holding high five and kindergarten registration in our auditorium on Thursday, May 19, 2022, from 8:00 AM to 10:30 AM. There will be staff available (including bi-lingual Somali, Oromo, and Spanish) to help you with the forms and to answer your questions. If you need another language, please contact the office. If you cannot come to Sullivan on this date, please come in sometime before June 24th during school registration hours (9:00 am to 1:00 pm) to complete your registration.
To complete the registration process, we will need you to bring the following information to Sullivan:
1. BIRTH VERIFICATION (birth certificate, passport, baptismal record, or immigration card)
2. RECORD OF IMMUNIZATIONS (including immunizations required for entry to Kindergarten)
3. Recent physical exam, if available.
4. Emergency contact information (names, addresses, phone numbers)
5. Information on childcare arrangements (address, phone number) for transportation purposes.
We will need all the items listed above before your child begins school. If your child attended preschool at Sullivan last year, we have this information on file, though you may need to bring an updated immunization record and a copy of their 5-year physical exam.
